Offline as in no Internet cable? You cannot get banned.
Offline as in plugged in but no xbox live? Debatable. Reports that some users have been banned have come in, but probably because they did something wrong on the settings side.

No unless it has been flagged perviously ie if it had a flash drive and Microsoft hadn't banned it on the last ban wave as long as you never plug in a Ethernet cable you will be fine and if your planning on going online your console will be banned within 4-6 hr if you're lucky

No, if you have no cables connected to your jtag then how could they catch you?
